Terms and Conditions for Claim Registrations
  1. MUIS reserves the right to request for the production of the relevant supporting documents for the exhumation of the deceased’s remains, including but not limited to the Claimant’s NRIC, Burial or Death Certificate of the deceased, or any other documents.
  2. MUIS shall be entitled to use and share the information provided by me (including any personal data, as defined in the Personal Data Protection Act 2012) in the course of my application made to MUIS with other government agencies or non-government entities authorised to carry out the relevant services in connection with the exhumation and reinterment works that forms the subject matter of this application, unless prohibited by legislation, to facilitate the processing and administration of such application and all related purposes thereto.
  3. For graves located at Blocks 23, 24, 25, 26, N-429, N-1-1 and N-1-2 within Choa Chu Kang Muslim Cemetery (“Principal Graves” and each a “Principal Grave”), if any written objection from any family member is submitted to MUIS, the claim will be voided and the deceased (“Principal Deceased”) will be exhumed as unclaimed (meaning the exhumation will proceed without a family claim).
  4. For registration to claim the remains of a deceased (“Supplementary Deceased”) exhumed from a grave other than a Principal Grave (“Supplementary Grave”), the Supplementary Deceased must have been buried for at least 15 years prior to the registration closing date.
  5. The Supplementary Deceased must be related to the Principal Deceased by way of one of the following relationships: (i) paternal or maternal grandparent or grandparent in-law, (ii) parent or parent in-law, (iii) sibling or sibling-in-law, (iv) child, grandchild or child-in-law or grandchild-in-law, (v) spouse, and (vi) child’s parent-in-law.
  6. For the exhumation of each Supplementary Deceased, a non-refundable fee of $80 (before GST) per grave shall be paid to Wareesan Management Pte Ltd. A copy of the Death or Burial Certificate of the Supplementary Deceased or a burial extract to be obtained from the National Environment Agency (NEA)’s Choa Chu Kang Cemetery Office for a fee of $20 (inclusive of GST) shall be produced to MUIS.
  7. Should any family member submit a written objection regarding the registration or exhumation of a Supplementary Grave, the registration will be cancelled, and the exhumation will not proceed.

  9. I will adhere strictly to MUIS’s schedule and timing for the exhumation in order not to affect the exhumation of other graves and the overall exhumation schedule.
  10. MUIS shall be entitled to reschedule the date of exhumation to another date if necessary to meet operational needs.
  11. On the scheduled day, if I am unable to witness any portion of the exhumation and reinterment process, I will provide written authorisation for MUIS to proceed on my behalf to the Customer Service Officers at MCEO.
  12. MUIS shall be entitled to temporarily suspend the exhumation works in the event a Category 1 Lightning Alert occurs for safety reasons, or exhumation works may be postponed to another day in the event of adverse weather.
  13. MUIS and/or its contractor or agent shall carry out any works deemed necessary for the smooth completion of the exhumation and reinterment works.
  14. I shall be bound by the prevailing Terms and Conditions by MUIS and such rules and regulations as may be prescribed by NEA governing the exhumation and reinterment of graves, where no monument or permanent structures are allowed to be erected on the reinterred grave. Erection of fencing, planting of trees/shrubs and placing of all other materials on and around the reinterred grave, including cementing on the turfed area between the reinterred graves is strictly prohibited.
  15. These Terms and Conditions shall be governed by and interpreted in accordance with the laws of Singapore.
  16. If any provision or part of any provision of these Terms and Conditions is or becomes invalid, illegal or unenforceable, it shall be deemed deleted, but that shall not affect the validity and enforceability of the rest of these Terms and Conditions.

The particulars given above are true to the best of my knowledge. I have carried out due diligence to ensure that there is no objection from my family members and relatives in regard to this claim. I hereby agree to indemnify and hold harmless NEA, MUIS and Wareesan Management Pte Ltd against any legal suit, claims, damages, losses, expenses or costs (including those asserted by third parties) arising directly or indirectly from the exhumation of the deceased(s) identified in this form. I also conscientiously declare the statements made by me in this form are true in every particular to the best of my knowledge, belief and ability.

 I acknowledge, understand and agree that should I be found to have knowingly provided false information, MUIS will investigate the matter and if warranted, initiate prosecution or take other action against me. I agree that the registration will be cancelled, and the exhumation will not proceed if I provide false information.
